DUTIES:

Provide coordination and management of service delivery for people with serious mental illness and/or intellectual/development disabilities, possibly with co-occurring substance use disorders. This position requires interactions with various disciplines, families, staff and supervisors as well as consumers. Requires collaboration with a multi-disciplinary team in assessment, diagnosis and person-centered planning to perform biopsychosocial assessments, develop individual plans of service, assess and monitor progress towards goals and objectives and maintain confidential records.

MINIMUM QUALIFICATIONS:

• Degree in Social Work, however, other Human Services degrees will be considered;
• 1 year experience working with those with intellectual/developmental disabilities and/or mental illness;
• Ability to work in Ontonagon County, Michigan, and travel to surrounding counties as needed;
• Strong interpersonal skills;
• Ability to work as a team member;
• Able to pass a criminal background check;
• Valid Michigan driver’s license and dependable vehicle.

DESIREABLE QUALIFICATIONS:

• Michigan Licensure as a Bachelor’s or Master’s Social Worker.
• Qualifications of a Child Mental Health Professional.
