DUTIES:

The Assertive Community Treatment (ACT)/Integrated Dual Disorder (IDDT) Team is a multi-disciplinary, community based, treatment team providing a broad range of supports to individuals with severe and persistent mental illness.

The duties for this position include providing co-occurring, crisis, and case management support to adult consumers, their families and natural support groups. This individual will perform assessments, develop treatment plans, manage referrals/linkage, monitor progress, integrate treatment, coordinate benefits and maintain the consumer record.

MINIMUM QUALIFICATIONS:

Bachelor’s degree or Master’s degree in social work, psychology, counseling or other human services field from an accredited program

Eligible for licensure with the State of Michigan

DESIREABLE QUALIFICATION:

One year experience with mental health services
